The publisher is also looking for people who, after completing the survey would consent to further follow-up questions via email or in some cases, a recorded telephone interview. Both of these situations however are optional. In addition, we will not eliminate any information you enter in this survey if you choose to remain anonymous.

Please also note that if you do consent to these interviews we will keep the source of our information confidential if you request us to do so. We also do not sell, share, rent, lend or otherwise distribute our survey results or email lists with any outside third parties
