On that page you can also download the Application Worksheet, which will make loading this Form simple.
There are 3 sections to this Form:
1. Information on Your Organization and Workshop Type
2. Your Workshop Plan (Open Ended Questions)
3. Organizational Background
Particularly for the longer questions in sections 2 and 3, we recommend that you cut and paste your answers directly from your Worksheet. And, don't forget to check the character limits!
*You will be able to return to this Form to edit your answers, but you MUST do so from the same computer and using the same web browser. And, cookies must be enabled.
However, if you are submitting multiple applications: To ensure that they get submitted properly you must do one of the following or the survey software will overwrite the first one you submit: 1) use a different browser for each one, 2) use a different computer, or 3) clear the cookies on the computer.
