Evantage Consulting is conducting a primary research with patients and caregivers who have used a Personal Health Record like Google Health or Microsoft HealthVault to manage a patients health information. You can read more about the project here http://tiny.cc/thxki

If you are you a patient or a caregiver and have used a Personal Health Record, please take this short survey to qualify for the research study. All participants who complete the study will be given $50 Barnes and Nobel e-gift certificate as a token of appreciation.
