Thank you for participating in the Workplace Behavior and Health Project. This study has been approved by IRB, eIRB # Pro00000603. The following questions concern your general health and habits, work and family experiences, and workplace behaviors. The survey should take no more than 20 minutes to complete.

Please be candid when you complete the questions. There are no right or wrong answers to any of the questions. Your responses will be averaged with the responses of other participants. All responses will remain confidential and individual responses will not be identified. However, to protect your rights, authorized research personnel, employees of the Department of Health and Human Services, the USF Institutional Review Board and its staff, and other individuals, acting on behalf of USF, may inspect the records from this research project.

There are no direct benefits or known risks to participating in this study. You are free to participate in this study or to withdraw at any time. Your participation or withdrawal does not have any associated risks. Your submission of this survey indicates your agreement to participate.
